Visit Request for Probationers/Parolees


THIS FORM IS ONLY FOR VISITORS WHO ARE ON PROBATION, ON PAROLE, SERVED TIME IN STATE PRISON OR BEEN IN CUSTODY IN THE PREVIOUS 90 DAYS. Other social visitors do not require pre-authorization.

Prospective visitors who are on parole or probation require the permission of the Facility Commander or Watch Commander prior to visiting. If you are on probation or parole, you may complete the following form to register your request. It will be directed to the appropriate staff member and you will be contacted by email or U.S. Mail with a determination.

Please note that it will take several days to investigate your eligibility to visit. Those requiring visit authorization should request it at least a week in advance (minimum).
